The purpose of this assessment is to help you take a proactive approach to your health by identifying potential risk factors and areas that may deserve further attention.

The “Time Out For Your Health” assessment includes a series of questions covering common men's health concerns, including heart disease, prostate health, low testosterone, depression, and other important health issues. Answer each question as honestly and accurately as possible. Your results are intended to help guide discussions with your doctor and support more informed healthcare decisions.

Basic Information

Depression

Depression is one of the most common and most overlooked health conditions affecting men. It can influence your mood, energy level, motivation, relationships, work performance, sleep, and overall quality of life. While depression is often associated with sadness, many men experience it through irritability, anger, fatigue, loss of interest, difficulty concentrating, or changes in sleep and appetite.

The following questions are designed to help identify symptoms that may be associated with depression. Please answer each question based on your experiences during the past two weeks and select the responses that best apply to you.

Depression may be diagnosed by a physician when symptoms persist for at least two consecutive weeks and affect normal activities.

Low Testosterone

Low testosterone is a common condition that can affect men as they age, although it may occur earlier in life as well. Testosterone plays an important role in energy levels, muscle mass, strength, bone health, sexual function, mood, and overall well-being. When testosterone levels decline, men may experience symptoms such as fatigue, reduced sex drive, weaker erections, decreased physical performance, difficulty concentrating, and changes in mood.

The following questions are designed to help identify symptoms that may be associated with low testosterone levels. Answer each question as honestly as possible based on your current health and recent experiences. If several of these symptoms apply to you, it may be worthwhile to discuss testosterone testing and treatment options with your doctor.

If you answer yes to the final two questions, or at least three other questions, ask your doctor whether testing is appropriate.

Benign Prostatic Hyperplasia (BPH)

Benign Prostatic Hyperplasia (BPH), commonly known as an enlarged prostate, is one of the most common urinary conditions affecting aging men. As the prostate enlarges, it can place pressure on the urethra and interfere with normal bladder function. This may lead to symptoms such as frequent urination, difficulty starting or stopping urination, a weak urinary stream, nighttime urination, and a sensation that the bladder is not completely empty.

The following questions are designed to assess the severity of urinary symptoms that may be associated with BPH. Please select the answer that best reflects your experience during the past month. Your responses can help identify whether further evaluation by a healthcare professional may be beneficial.

Prostate Cancer

Prostate cancer is one of the most common cancers affecting men, particularly as they grow older. While many cases develop slowly and may not cause symptoms in their early stages, some forms can be aggressive and life-threatening if not detected and treated promptly. Early identification through appropriate screening and medical evaluation can significantly improve outcomes.

The following questions are designed to help identify risk factors and symptoms that may be associated with prostate cancer. Please answer each question as accurately as possible and select the responses that apply to you. If you have concerns about your results, discuss them with your doctor and ask whether prostate cancer screening is appropriate for your age, family history, and overall risk profile.

Men age 50 and over, and African American men age 40 and over, should talk with a physician about prostate-specific antigen screening and prostate exams.

Heart Disease

Heart disease remains one of the leading causes of death among men and often develops silently over many years before symptoms become obvious. Factors such as high blood pressure, elevated cholesterol, diabetes, smoking, excess weight, poor diet, lack of exercise, chronic stress, and family history can significantly increase a man's risk of developing cardiovascular disease.

The following questions are designed to help identify common risk factors associated with heart disease. Please answer each question honestly and select the responses that apply to you. Understanding your risk profile can help you have more informed conversations with your doctor about prevention, screening, and lifestyle changes that may improve your long-term heart health.
