TAKE TIME OUT FOR YOUR HEALTH

Stroke Risk Assessment

This assessment is designed to help identify common stroke risk factors, warning signs, lifestyle concerns, and vascular health issues. Your results can help you have a more informed discussion with your healthcare provider about stroke prevention and risk reduction.

Answer each question as honestly and accurately as possible. This assessment is intended for educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment.

Basic Information

Race / Ethnicity

Personal & Medical Risk Factors

This section focuses on medical conditions and personal characteristics known to increase stroke risk. Some factors cannot be changed, while others can be managed through proper medical care, medication, and lifestyle changes.

Answer each question based on your current health status and medical history. If you are unsure of an answer, choose the response that most closely reflects your situation.

Lifestyle Risk Factors

Many stroke risks are linked to everyday habits. This section reviews lifestyle factors that may contribute to high blood pressure, blood vessel damage, cardiovascular disease, and long-term stroke risk.

Answer these questions based on your typical habits and lifestyle during the past year. Honest answers will provide the most useful assessment results.

Current Warning Signs & Symptoms

Some symptoms may indicate reduced blood flow to the brain or another serious medical condition that requires immediate attention. Recognizing these warning signs early can dramatically improve outcomes and reduce the risk of permanent disability.

If you are currently experiencing sudden weakness, numbness, facial drooping, confusion, difficulty speaking, vision changes, dizziness, or a severe headache, seek emergency medical attention immediately. Do not wait for symptoms to improve on their own.

Blood Pressure & Vascular Health

High blood pressure is the single most important modifiable risk factor for stroke. This section focuses on blood pressure awareness, vascular health, preventive screening, and cardiovascular risk factors that may increase the likelihood of stroke.

If you do not know your blood pressure numbers, cholesterol levels, or cardiovascular risk status, answer as accurately as possible. Regular medical checkups and preventive screenings can help identify problems before symptoms occur.

Review Your Results

This assessment does not store your name, email address, phone number, or street address. Results are stored without direct personal identifiers and may be used only for anonymous reporting and research purposes.

For concerns, consult your doctor or another qualified healthcare professional.
